I wish to participate in the Joy Ride, which I understand to be a multi-day, non-competitive bicycle pledge ride and related training rides and other events (collectively hereafter " the Joy Ride "). I understand that in participating in the Joy Ride I will be using public streets and facilities where many hazards exist, and I am aware of and understand the risks and associated incidents which may result. I am also aware that accidents occur during bicycle riding due to the negligence of others and that I may be seriously injured or killed as a result. I am voluntarily participating in this Event as a Rider or a Volunteer Crew Team Member with knowledge of the dangers involved, and I agree to accept any and all risks of injury or death.

In consideration for being permitted to participate in the Joy Ride, I agree to assume all risks and to release and hold harmless FAB Adventures, Inc, ACE Ride Productions, EPIC (Empath Partners in Care), Miracle of Love, Compass Community Center, Pridelines, and Broward House including board members, officials, employees, volunteers, participating organizations (and all of their respective officers, directors, agents, employees and members)(collectively all of the above persons and entities shall be referred to as the "Released Parties" hereafter) who, through negligence, carelessness or any other cause, might otherwise be liable to me.

I confirm that I am physically capable of participating in the Joy Ride, and my medical care provider has approved my participation. If I am aware of or under treatment for any physical infirmity, ailment or illness, my medical care provider knows of and has approved my participation in the Joy Ride. I acknowledge that I, and I alone am solely responsible for my personal health and safety, the condition of the bicycle I will be riding, and the personal property I bring with me. I acknowledge that the medical insurance information I have provided on the Medical Form is current and complete and that I am solely responsible for procuring and maintaining all medical insurance I deem necessary and that the Released Parties have recommended that I procure and/or maintain medical insurance. I accept full responsibility for any costs incurred for medical treatment due to failure to procure or maintain insurance, or providing outdated, or falsified insurance information. I understand that it is ultimately my responsibility to provide payment to any hospital/emergency response technicians/emergency transport company that may provide services to me as a result of injury/illness during the Joy Ride.

I will read the Joy Ride description and rules for participation in the Joy Ride, and I will abide by all rules and regulations established by the Joy Ride organizers and personnel as well as the state vehicle code. If I am participating in the Joy Ride as a rider, I agree to wear a properly fitted and adjusted CPSC, ANSI, ASTM or SNELL-certified helmet during the Ride at all times. I further agree that my participation in the Joy Ride is subject to the sole discretion of the organizers and Medical Director of the Joy Ride, and that my participation in the Joy Ride may be limited for medical or other safety-related reasons.

I intend by this Waiver and Release to release, in advance, and to waive my rights and discharge each and every one of the Released Parties from any and all claims for damages for death, personal injury or property damage which I may have, or which may hereafter accrue to me, as a result of my participation in any aspect of the Joy Ride, even though that liability may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective property or equipment owned, maintained or controlled by them or because of their possible liability without fault. Additionally, I covenant not to sue any of the Released Parties based upon their breach of any duty owed to me as a result of my participation in any aspect of the Joy Ride. I understand and agree that this Waiver and Release is binding on my heirs, assigns and legal representatives and that the Released Parties shall be exempt from liability to me, my heirs, assigns and legal representatives.

I understand that my name, photograph, voice, or likeness may be used by any one of the Released Parties and their licensees, affiliates and employees. I consent to and authorize, in advance, such use and waive my rights of privacy or publicity I have in connection therewith.

I have carefully read this Waiver and Release and fully understand its contents. I certify that I am at least 18 years of age. I am aware that this is a RELEASE OF LIABILITY, COVENANT NOT TO SUE and a contract between me and the persons and entities mentioned above and all of their respective officers, directors, employees, agents and representatives and I sign it of my own free will.
